Your UDI should follow some requirements listed on the MDR 2017/745. To help you fulfill these requirements, I create a UDI checklist that you can use to verify that your product is compliant.
This list should be used for each product you have on your portfolio. A cover page is included with an identification of the products.
It contains also a approbation table table that can help you to confirm that all the information you included were reviewed.
